1911 : le pays est touché par une canicule. Comme lors des grandes crises (guerres, épidémies), la population hospitalisée est frappée par un surcroît de mortalité sans que nul ne s'en émeuve hors de l'institution hospitalière. Aucun ministre ne démissionne, à un moment où il n'y a pas de ministère spécifique de la Santé publique. 2003 : le pays est touché par une canicule. C'est de l'hôpital que partent les premiers appels pour alerter la population et, quelques mois plus tard, le ministre de la Santé publique doit démissionner. Jadis objet d'indifférence, l'hôpital est devenu un thème central de l'action politique. De la fin du XIXe au début du XXIe siècle, l'hôpital connaît une formidable mutation. Réservé autrefois aux indigents, il accueille aujourd'hui l'ensemble de la société. Alors que l'on y allait parcimonieusement, s'y rendre est désormais un réflexe. A l'entrée d'un établissement hospitalier, les malades étaient dépossédés de leurs effets personnels, tandis que les droits des patients sont aujourd'hui systématiquement rappelés. Hier lieu d'accueil mais de relégation, l'hôpital est devenu un endroit où s'exerce la citoyenneté. L'on soignait mal et l'on guérissait peu dans l'hôpital des débuts de la IIIe République ; les seuls médecins présents étaient le plus souvent encore étudiants et le mot | infirmière | désignait un personnel sans formation. C'est dans les hôpitaux que les Français, sous la Ve République, sont le plus sérieusement pris en charge lorsque les frappent la maladie ou l'accident ; les meilleurs praticiens y exercent et le personnel paramédical est particulièrement qualifié. Dans sa démarche, l'auteur ne néglige pas l'évolution de la médecine, de l'économie et du droit hospitalier, et a recours à la sociologie des professions et des institutions. Pour comprendre comment l'hôpital est devenu ce formidable outil au service de la population, il prend en compte tous les acteurs : les médecins et le personnel des services de soins bien sûr, mais aussi le personnel administratif et les directeurs sans oublier les ouvriers qui ont, jusqu'au milieu du siècle, tenu dans le monde hospitalier un rôle de premier plan.
